What to bring to your appointment.
Please bring with you
The more complete your documents are, the better we can assess your situation — and the more likely we are to spare you repeat examinations.
- your insurance card and the referral slip from your general practitioner
- an up-to-date list of all medication you take or inhale (your medication plan is ideal)
- relevant doctors' letters and hospital discharge reports
- results of previous allergy tests
- cardiology or rheumatology findings, if available
- chest imaging — X-ray and CT scans, on CD if possible
- laboratory results including full blood count and total IgE
- your inhalation devices, so that we can check how you use them together
